Apply Now to Join New Social Care Network

Earlier this summer, Healthy Alliance was appointed as the North Country region’s Social Care Network (SCN) under the state’s 1115 Waiver Amendment. Healthy Alliance is now accepting applications from organizations interested in partnering in their SCN either as a contracted Health-Related Social Needs (HRSN) service provider or as part of Healthy Alliance’s broader network of providers. These applications will be reviewed and evaluated by Healthy Alliance using established criteria with a focus on an applicant’s ability and/or willingness to meet the SCN and HRSN Program requirements under the 1115 Waiver Amendment. While Healthy Alliance will build its network over time using a rolling application, we urge Get Healthy North Country Network members to apply ahead of the first deadline: Sept. 30.

Please note: If your organization is already working with Healthy Alliance (sending/receiving through their referral network, working with an assigned Healthy Alliance Performance Consultant, etc.), you still need to complete an application to participate in Healthy Alliance’s SCN under New York’s 1115 Waiver Amendment’s SCN & HRSN Program requirements. 

We need members of the Get Healthy North Country Network to get involved now. It’s the best way for the North Country to ensure it has a strong voice in the development of this region’s social care network and, in turn, make sure that our residents have the support they need to live their healthiest lives.

Do not wait for Healthy Alliance to reach out to your organization. You must be proactive and apply before the Sept. 30 deadline.

Again, we cannot stress enough how important it is to apply and get your organization enrolled. It is also critically important that North Country organizations participate in upcoming Healthy Alliance meetings. Most importantly, we urge you to apply before the first deadline on Sept. 30.
