Get Healthy North Country (GHNC) is a proven community care hub (CCH) that specializes in chronic disease prevention and self-management. It helps organizations participating in performance-based contracts achieve outcomes they are now being held accountable for but cannot deliver alone. Federal policy, CMS innovation models, and rural health investments are converging on a single truth: chronic disease prevention and self-management must happen in communities, not just clinics. GHNC already operates in that space at scale. It offers a regional prevention infrastructure, purpose-built for the value-based future of rural healthcare.
